Most people think of stress primarily as a mental experience. Something that happens in your head, affects your mood, and maybe gives you a headache or an upset stomach if it gets bad enough.
It is considerably more physical than that.
Chronic stress changes your body’s tissue. It alters your hormone function, raises your pain sensitivity, and leaves a measurable imprint on your muscles, fascia, and nervous system. By the time most people seek treatment for the physical symptoms, the stress driving them has often never come up.
What Stress Does to Your Body
When you encounter a stressor, your body activates the HPA axis, triggering the release of cortisol and adrenaline. In the short term this is adaptive and useful. Your body mobilizes energy, sharpens focus, and prepares you to respond.
When the stress response fires repeatedly over weeks, months, or years, the same hormones that help you in the short term start causing damage.
Chronic cortisol exposure leads to muscle wasting, pain, and widespread musculoskeletal dysfunction. It also degrades connective tissue. Research has shown that chronic cortisol production breaks down collagen in the fascia, the web of connective tissue that surrounds and connects every muscle, organ, and structure in your body. Less collagen means stiffer, less resilient tissue that is more prone to injury and slower to heal.
Cortisol dysfunction becomes directly implicated in chronic pain and inflammation over time. The HPA axis, exhausted from chronic activation, loses its ability to regulate the inflammatory response properly. Pain that should resolve starts to persist. Injuries that should heal don’t.
What Stress Does to Your Fascia
When you are under stress, your body unconsciously braces. Jaw, chest, abdomen, upper back, shoulders. You probably already know which areas are yours. Over time, that chronic bracing causes the fascia in those areas to become rigid and restricted, limiting movement and contributing to pain. This is not a metaphor. Stress hormones directly influence the fibroblasts and myofibroblasts that maintain fascial tissue, causing it to stiffen and lose elasticity.
People with chronic stress and depression show measurably increased fascial stiffness and reduced elasticity compared to controls. The body molds itself around its stress state over time.
The TCM View
Chinese medicine has a framework for this that predates the cortisol research by a few thousand years. 😉
Chronic stress in TCM is primarily a Liver pattern. The Liver is responsible for the smooth flow of Qi throughout the body, and prolonged emotional strain causes that flow to stagnate. Liver Qi Stagnation manifests as tight muscles (particularly the upper back, jaw, neck, and shoulders), digestive disruption, irritability, headaches, and menstrual irregularities, among other things.
What the research describes as HPA axis dysregulation and fascial restriction, Chinese medicine has been describing as Liver Qi Stagnation for centuries. Different language, same pattern.
Over time, stagnation can generate heat, consume Yin, and affect the Heart and Kidney systems as well. Chronic stress left unaddressed tends to get more complex, not less. This is why treating it early matters.
What to Do About It
If stress has had time to become structural, managing your stress load alone may not be enough. You also need to address the tissue.
On the stress management side: acupuncture, mindfulness, deep breathing, movement, and sleep all help regulate the HPA axis and move Liver Qi. These are the tools with the best evidence for actually changing the underlying physiology.
On the tissue side: acupuncture, cupping, and bodywork directly address fascial restriction, release trigger points, and restore movement in areas that have been chronically braced. Herbal medicine can support the process from the inside.
